What Youll Do:
To act as a liaison between the home, school and community agencies for students who are determined to be at risk for academic failure. Develop and facilitate an action plan between the school and the family to help ensure students success to include weekly in-school contacts with children and bi-weekly home visits with parents. Act as the attendance and truancy liaison for individuals on the referral caseload list and others as requested.
Responsibilities
  • Update direct supervisor on an ongoing basis regarding concerns, issues and caseload;
  • Develop and enhance communication, networking, and collaboration between parents, schools, and community agencies;
  • Provide supportive services and follow-up activities both in the home and school environments;
  • Upon receipt of a referral, convene a meeting of all involved parties to cooperatively develop an action plan and schedule the first home visit;
  • Schedule and keep home visits with parents on a minimum bi-weekly basis to gather information, facilitate goal setting and assist in problem solving;
  • Promote strategies for parent/guardian involvement in school related activities;
  • Foster a positive and open working relationship between parents and the school;
  • Meet with teachers weekly in the school setting to assess progress and make necessary adjustments in action plans;
  • Meet weekly with referred students in the school setting;
  • Represent the agency, school district and program in a professional manner at all times;
  • Seek consultation and make referrals to community agencies as needed;
  • Provide services coordination for families who have involvement with multiple agencies;
  • Maintain required records and documentation for caseload and informal support tasks, including Formstack data entry as required by supervisor;
  • Attend staff meetings and trainings as required by supervisor;
  • Participate in inter-agency groups and committees when possible;
  • Act as truancy liaison in your individual building(s) to ensure students are attending on a regular basis, participate in Preliminary Inquiries and court hearings on an as needed basis;
  • Act as a facilitator for any classroom presentations on an as needed basis;
  • Act as a facilitator for any groups (i.e., divorce, friendship, COA, etc.) on an as needed basis.

  • B.A. or B.S. in Social Work, Guidance and Counseling, Sociology, Psychology or related human service field is required. 1 -2 years experience working with school age children preferred. An education degree with appropriate course work and/or experience will be considered.
