You won't just process paperworkyou'll case manage futures, ensuring every person has the support and resources to achieve their goals.
- Facilitate & Develop the Plan:Lead assessments and develop the Individual Service Plan (ISP)a roadmap that reflect each persons goals and needs.
- Keep the Plan Fresh:Regularly review and update ISPsto ensure they always reflect the individuals current needs and goals.
- Act Fast in a Crisis: Guaranteeimmediate, 24/7 service access for urgent situations and participate in the on-call rotation(divided by the whole team) to manage emergencies and ensure safety.
- Check for Progress:Implement ongoing service reviews to confirm that services are appropriate and outcomes are being met.
- Coordinate Success:Provide continuous case managementfocused on achieving the desired outcomes defined in the ISP.
- Keep Records Tight:Maintain organized case files and case notes with precision and timeliness.
- Partner with Providers: Train providerson the ISP and ensure they meet all service expectations.
- Secure the Funding:Complete cost projections and documents quickly to authorize necessary servicesand funding.
- Protect Health and Safety:Take immediate actionto resolve any health or welfare concerns.

***This job requires travel throughout Greene County and would be a mix of in office, remote work and in-person visits.***
Minimum Qualifications:
- Bachelor or higher degree required.
- Case management experience.
- Strong communication, organization, and problem-solving skills.
- Ability to work collaboratively with individuals, families, and service providers.
- Must successfully pass State and/or Federal Criminal Background Check, ODODD Abuse Registry, Nurse Registry, Office of Inspector General, Sex Offender Registry, Offender Registry, and SAM Database required by law or rule.
- Must successfully complete pre-employment physical including drug test.
- Must obtain and maintain certificationidentified for the position by the Ohio Department of Developmental Disabilities (at hire).
- Must maintain valid Ohio drivers license in accordance with Board policy.
