• Clinical Social Work License in state of Oregon REQUIRED! OR... Master's degree from a school of Social Work accredited by the Council of Social Work Education.
• Associate working towards Clinical Social Work licensing requirements for the State of Oregon with one year experience in a health care setting.
• State of Oregon licensure as a Clinical Social Worker is required within two (2) years of employment.
• Department: CM (Case Management) &/or UR (Utilization Review)
• General Certification Requirements (BLS, etc.)
• "Other Certifications/Specialty certifications requirements? LCSW CCM (Certified Case Manager), ACM (Accredited Case Manager), C-SWCM (Certified Social Work Case Manager), CBHCM (Certified Behavioral Health Case Manager)": Not required Case load/# patients per day/consults per day: 15-20, Varies based on need EMR: Meditech
• Do you require InterQual criteria or Milliman experience: No
• Is this position for UR, discharge planning, or both? DC planning and clinical social work
• What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.): Inpatient and ER
• Common types of patients & job duties: care management for transition of care planning including home, HH, SNF, IRU, LTAC, hospice, etc. Social work psychosocial assessments for safety planning on inpatient psych, assist with difficult placement and bio-psychosocial needs of patients, provide resources and collaboration with community partners to connect patients with needed services
• Experience or 'Must have' skills: Clinical Social Work License in state of Oregon, and Acute hospital experience
• CM Manager/Supervisor: No
• Shifts & Scheduling? Monday - Friday 8-4:30
• Weekend Requirement: Possibly as needed
• Holiday Requirements: Rotation, depends on coverage
• Scrub Color/Dress Code: Business casual
• RT for DC Planning? No
• Pharmacy for DC Planning? No

• ACLS
• Clinical Social Work License in state of Oregon, and Acute hospital experience
