Pay Rate: $60/hr on W2 or $64/hr on 1099
Position Overview:
We are seeking a Licensed Master Social Worker (LMSW) to join our Case Management team at Providence Health and Services.
This role involves coordinating care in an acute care setting, working closely with an interdisciplinary team to ensure that patients receive comprehensive care. The ideal candidate will have experience in case management and will be comfortable floating between various units within the hospital, including the emergency department, ICU, pediatric, and specialty units.
Key Responsibilities:
- Provide case management services across a range of patient care units, ensuring coordinated and effective care.
- Collaborate with nursing staff, physicians, and other healthcare providers to develop comprehensive care plans.
- Float between the Colby Campus and possibly the Pacific Campus (1.7 miles away) based on department needs.
- Perform patient assessments and provide counseling, resources, and referrals as needed.
- Work with patients and families to ensure clear communication and support throughout treatment and recovery.
- Assist with discharge planning and ensure patients have access to community resources for continued care.
- Utilize EPIC for patient charting and documentation.
Required Qualifications:
- Licensure: Valid LMSW (Licensed Master Social Worker) license in the state of Washington.
- Experience: Minimum of 2 years of case management experience, preferably in an acute care facility.
- Certifications: BCLS (Basic Cardiac Life Support) required.
- Charting Experience: Experience with EPIC charting system is required.
- Flexibility: Must be willing to float between different units within the facility and potentially to nearby sister facilities.
Additional Information:
- Travel Requirements: Candidates will be required to float within the hospital and to the nearby Pacific Campus as needed.
- Local Candidates: Local candidates are not accepted for this position.
