We are hiring a Social Worker for one of our clients in Hamilton, NJ.

Job Description:

  • Review new funding year, continuation of service and amended service plans to ensure case managers submit detailed, comprehensive and complete service plans
  • Work with case managers on plan updates, revisions
  • Review the TBI Fund regulations to confirm eligibility and appropriateness of service requests
  • Reference TBI Fund provider database to confirm accuracy of service provider rates on service plans
  • Make service plan recommendations for TBI Fund action
  • Prepare abstracts of service plans monthly to be presented for consideration to the TBI Fund Review Committee
  • Redact supporting documents to accompany abstracted service plans (to protect client information)
  • Participate in monthly TBI Review Committee meetings
  • Forward abstracts with recommendations to Administrative Staff for decision letter preparation
  • Address issues, problems and complaints and respond to consumer inquiries (telephone, email or I&R referral) and provide excellent customer service
  • Identify and resolve problems in the delivery, administration, implementation or integration of program activities
  • Respond to, and coordinate requests from internal units, individuals and other agencies to provide information concerning program activities
  • Prepare letters, memos, instructions, transmittals and other information for approval and signature
  • Conduct site visits and prepare reports
  • Gather and review information from applicants
  • Explain eligibility criteria for program services
  • Review and evaluate personal or financial data to determine client eligibility
  • Prepare reports, correspondence, and statistical material, as needed/requested
  • Represent the TBI Fund/DDS at public meetings and conferences
  • Maintain essential records and files, including client files
  • Retrieve invoices; ensure invoice is complete; verify type of service, award dates, and availability of funds, and calculations.
  • Communicate with service providers, as needed, to obtain missing or incomplete invoice information
  • Create vouchers using appropriate forms; review and save completed vouchers to Completed Vouchers online folder.
  • Track vouchers to Fiscal for payment.
  • Initiate internal fiscal investigations to resolve vendor issues.

Case Manager Job Responsibilities:

  • Designs implement and monitor individual service plans.
  • Counsel and aid individuals and families requiring assistance.
  • Maintains beneficiary's records according to professionally accepted standards
  • Complete required documentation. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs.
  • Coordinate beneficiary's services with service provider, as needed.
  • Network with service providers to generate community resources for participants, as needed.
  • Assess beneficiary's needs annually in person; quarterly by phone or virtually
  • Design, implement and monitor individual service plans
  • Counsel and aid individuals and families requiring assistance, as needed
  • Complete documentation, including but not limited to data entry, progress notes, activity reports, time sheets and logs
  • Maintain records according to professionally accepted standards
  • Coordinate beneficiary's services with service provider(s), as needed
  • Network with service providers to generate community resources for beneficiaries, as needed.
  • Skills required:
  • Social workers or health/human services-related professionals for case management for the Traumatic Brain Injury Fund (TBI), a program providing services and supports to individuals with a medically documented TBI.

Work hours:

  • 9:00 AM-5:00 PM
  • Lunch Duration: 1 hour (Unpaid)
