URGENT POSITION
Salary Range: $30 - $40/hour (dependent on experience)

• Collaborate with a multidisciplinary team to develop and implement individualized care plans.
• Advocate for patient needs and connect them with appropriate resources.
• Maintain accurate documentation and uphold confidentiality standards.

Requirements:

• Master's degree from an accredited school of social work - Required
• Current or eligible for required state licensure - Required
• Meets state-specific requirements, such as LCSW - Required
• Minimum of 1 year of clinical social work experience, preferably in an ESRD setting - Required
• Current CPR certification - Preferred
