On Call, Call Back Requirements: none

Weekend Rotation: none

Holiday Requirements: every other holiday required

Floating Requirements: to all inpatient areas as needed

JOB DESCRIPTION

To be minimally qualified for this opening, your candidate MUST HAVE:
- 1 year Social Work experience
- MUST have experience in Cardiac and Stroke patients
- active BLS and MSW in hand at time of submission
- EPIC experience
HIGHLY PREFERRED: Experience with Careport and Passar

Additional Skills required: Discharge planning, anticipating needs, and care coordination.

Traveler can expect to work with patients for Social Work needs (primarily Cardiac and Stroke patients) - will be sharing assignments with RNs. Will have Other Social Workers, Case Management, RNs, Coordinator, and Leadership for support while on contract. Will receive UP TO 1 week for orientation, based on comfort level.
