AdvisaCare Home Health & Hospice is seeking a dedicated In-Home Social Worker to become a part of our compassionate team in the community. As a valuable member of our multidisciplinary team, you will conduct assessments, provide emotional support, and assist clients and their families in navigating their care needs within their home environment.

Your role will focus on enhancing the well-being of our clients by addressing psychosocial needs, advocating for resources, and ensuring that our clients receive the highest quality of care.

Requirements

• Master's degree in Social Work (MSW) from an accredited school
• Current state licensure to practice as a Social Worker (LMSW or LCSW)
• Experience in home health care or hospice preferred
• Strong assessment, communication, and organizational skills
• Valid Driver's License, car insurance, and reliable transportation

Key Responsibilities:

• Conduct psychosocial assessments of clients and their families in their homes
• Develop and implement goal-directed care plans
• Provide individual and family counseling as needed
• Connect clients and families with community resources
• Document all services provided and maintain accurate client records
• Collaborate with other healthcare professionals to ensure comprehensive care
• Participate in staff meetings and contribute to team discussions in identifying client needs
