Overview
The Start of Care Registered Nurse is responsible for completing patient admissions and start-of-care assessments in the home health setting. This role focuses on ensuring a smooth transition into care, establishing comprehensive care plans, and coordinating with the interdisciplinary team to provide exceptional patient outcomes. The position offers a structured weekday schedule and opportunities for additional compensation through optional on-call duties.
Key Responsibilities
Conduct comprehensive start-of-care and admission assessments for new patients
Develop individualized care plans in coordination with physicians and other healthcare professionals
Provide skilled nursing services and patient education in the home environment
Ensure all documentation meets regulatory and organizational requirements
Monitor patient progress, reassess care needs, and communicate updates to the care team
Support patients and families in understanding the plan of care and available resources
Maintain accurate and timely records of all patient interactions and interventions
Participate in team meetings, case reviews, and quality improvement initiatives
Adhere to clinical policies, safety protocols, and home health regulations
Schedule and Shift Details
Monday through Friday, 10 AM to 6 PM (start and end times may vary day to day)
Optional on-call shifts available for additional earnings
Travel
Local travel required across multiple counties for home visits and admissions
Qualifications Required
Active Registered Nurse (RN) license in Illinois (IL)
Minimum 1 year of hospice or home health experience
Management or leadership experience preferred
Current certifications: Basic Life Support (AHA BLS) and Advanced Cardiac Life Support (AHA ACLS)
High School Diploma or GED required; nursing degree preferred
Valid driver's license and reliable transportation
Preferred
Experience with electronic medical record (EMR) systems
Strong knowledge of home health regulations and compliance standards
