Duties

Provides direct patient care which includes but not limited to psychosocial assessments, case management, care coordination, and discharge planning.


Must use a high level of skill in assessing the complicated bio-psychosocial problems of Veterans admitted to the inpatient units.


As a part of an interdisciplinary team, assess the bio- psychosocial stressors that have contributed to a patient's hospitalization; assists in the formation of the treatment plan; and develops a carefully thought out plan for treatment and discharge.
Conducts meetings with patients and their families, and collaborate with outpatient providers and community agencies to realize a plan that utilizes the patient's strengths and existing supports.


Serves as the program coordinator and is administratively responsible for psychosocial program providing services to patients in the ICU, Surgery Unit, and Transplant Services.


Has supervisory and administrative responsibility for the management of the assigned social workers who function within the ICU, Surgery Unit, and Transplant Services.


Assists in planning, establishing, and implementing policies and procedures; developing individual or group goals and objectives, as well as monitoring, operating, evaluating, coordinating, and overseeing the Social Work staff.
The ICU, Surgery Unit, and Transplant Services Social Work Supervisor will assure the Social Workers whom they supervise are aware of and adhere to all current directives of VHA, and meet current JCAHO standards, State licensure standards, and VA-OIG expectations.
