Support ConsultantSalary: $50-55k/yearLocation: St. Paul, MNSchedule: M-F, Daytime hoursPosition OverviewAs a Support Consultant, you will work with individuals enrolled in the program to educate them on program models, rules, processes, and policies. You will assist with creating service delivery plans and quality review plans, and help evaluate service utilization to provide recommendations for service improvements.Key Responsibilities:Conduct home visits to a diverse range of participants, ensuring program adherence and providing guidance.Spend at least 70% of work hours on home visits, potentially exceeding this as business needs dictate.Organize caseload effectively to maximize efficiency in driving time and service delivery.Drive 60 miles or more to visit participants' homes, ensuring a reliable and safe driving record.Master internal systems and tools to maintain accurate records and ensure compliance.Core Skills:Strong organizational and time management skills to independently manage a caseload.Ability to prioritize tasks in a dynamic and fast-paced environment.Excellent verbal and written communication skills.In-depth understanding of self-direction principles and ability to communicate these to others.Proficiency in Microsoft Office and comfort using technology for documentation and reporting.Team-oriented with the ability to collaborate and communicate effectively in field and office settings.Timely documentation of home visit notes, ensuring proper upload on the same day.Professional demeanor and ability to represent the company positively to participants and stakeholders.QualificationsEducation & Experience:2-3 years of experience working with vulnerable populations in their homes.BA or BS in social work, sociology, psychology, or related field preferred.1-2 years of experience in social services, healthcare systems, or related state and federal programs (e.g. Medicaid, Medicare).Familiarity with electronic records is preferred.Required Resources:Valid driver’s license with a satisfactory driving record (Mileage reimbursement provided).Reliable personal automobile.Stable internet connection and a secure workspace.If you are passionate about making a difference in the lives of individuals with unique needs and enjoy working in a dynamic, team-oriented environment, we invite you to apply!
