Supportive Home Care Workers needed in Stevens Point, WI
Assignment 1: 10 hours/week. Mileage reimbursement. Must be able to transport.
Assignment 2: 2.5 hours/week. Mileage reimbursement. Must be able to transport.
Assignment 3: 5 hours per/week. Mileage reimbursement. Must be able to transport for medical appointments.

CARE WORKER: Provide confidential services to physically or developmentally disabled individuals and/or frail elderly consumers in their home with the goal of maximizing their independence for as long as possible.
DUTIES: The SHCW will be trained in the provision of supportive home/personal care services and in each skill that is assigned which can include but not limited to:
Providing companionship
Activities such as game/card playing and walks based on consumer preference
Assistance with bathing
Assistance with toileting including use of bedpan, urinal, commode or toilet
Assistance with dressing and undressing
Assistance with mobility and ambulating including use of walker, cane and crutches
Assistance with simple transfers including bed to chair or wheelchair and reverse
Changing of consumer(s) bed, laundering of bed linens and consumer(s) personal clothing
Skin care (excluding wound care)
Care of eyeglasses and hearing aids
Light cleaning in essential areas of home during personal care activities
Meal preparation and meal serving
Provide errand services and/or transport consumer
Promptly reporting changes in the consumer's condition to the designated supervisor
Utilize Electronic Visit Verification (EVV) when required
Completion and submission of time sheets/documentation in a thorough, accurate, and timely manner
Maintain confidentiality of consumers information
Adhere to and abide by all policies and procedures
