Care ManagerBlue Ridge Health is currently seeking a Care Manager to be part of our Tailored Care Management Team in Western North Carolina. The successful candidate will be accountable for the medical, dental and behavioral health care management of the member. Responsibilities include:Evaluate existing, new and prospective members based on their needs & desiresMaintain constant communication with members while addressing their concerns and goalsResponsible for the four key components that make up successful case management: Intake, Assessment, Service Planning, Monitoring and Evaluation.Compliance with regulatory bodies and in-house clinical guidelinesBuild rapport with members, their families and support systems while collaborating with the health care teamDevelop care plans for members and provide support as neededMay be responsible for supervisory tasks for Care Management Extenders in concert with Care Management SupervisorWhat We're Looking For:A Qualified Professional with Mental Health experience is required that may include either a license, provisional license, certificate (such as a CADC), bachelor's degree (with Two - four years of experience meeting required definitions) or a Registered Nurse.Experience in care managementKnowledge of care management principles and reimbursementEffective listening and communication skillsExperience with psychological aspects of careExcellent organizational and time management skillsBilingual preferredExperience with Electronic Medical Records and Case Management PlatformsAbout Blue Ridge Health:At Blue Ridge Health our mission is to improve Health, inspire Hope, and advance Healing through access to Compassionate, Affordable, and Quality Care.
