Case Coordinator (Home Care) Temporary (Possible Temp-to-Perm)
Location: 520 8th Avenue, New York, NY 10018
Schedule: Monday Friday | 9:00 AM 5:00 PM | Fully On-Site
We're seeking an experienced Case Coordinator to join our client's Home Care team for an immediate temporary assignment. This position will run from November through the first week of January, covering for a staff member on medical leave. There's a strong possibility of transitioning to a temp-to-perm role should the incumbent not return.
This is a non-clinical, fully in-person position responsible for coordinating and scheduling Home Health Aides (HHAs) to ensure clients receive timely, high-quality home care services in accordance with each plan of care.
Key Responsibilities-
Coordinate and schedule home care services to ensure client needs and authorizations are met.

Collaborate with internal teams to select appropriate home care workers based on client requirements.

Communicate effectively with clients, referrers, clinicians, and agencies regarding service updates or changes.

Maintain accurate case documentation and update records within internal systems (HHA Exchange).

Provide support and guidance to home care workers, ensuring adherence to care plans and agency policies.

Participate in case conferences and address scheduling, attendance, or payroll reconciliation issues.

Report service issues or unusual occurrences to supervisors promptly.

Participate in rotational on-call responsibilities as assigned.

1+ year of HHA Exchange experience required

Prior Case Coordinator experience required

Home Care experience required

Experience scheduling HHAs required

High School Diploma or GED required; Bachelor's degree preferred

Minimum of 2 years of related experience in home care, health, or community services

Excellent communication, organization, and problem-solving skills

Strong teamwork and customer service orientation

Business office environment; phone and computer use required.

Mandatory annual trainings may include Sexual Harassment Prevention, HIPAA, OSHA, and IT Security.
