Some conditions that may benefit from physical therapy include sports injuries, amputations, joint replacement, back and neck pain, headaches and neurological conditions.

A physical therapist can help you:

• Return to normal daily activities

• PT Experience - Required
• Outpatient Experience - Required
• Women's Health Experience - Preferred

MUST notate if T has EPIC exp in profile (please notate if within last year or not)

Profile Requirements:

• Work History
• Skills Checklist, current to 1 year, pertinent to specialty
• 2 Supervisory Reference Forms, taken within the last two years
• Licensure & Certifications (MI PT license & BLS - AHA)
• Candidates Permanent Address (Facility will not consider local candidates - 50 mile radius restriction)

CERTIFICATION REQUIREMENTS :

• BLS (AHA)
