THIS IS FOR A 6 WEEK ASSIGNMENT ONLY AND FOR AN ASAP START OF 9/27 OR 9/30 ONLY! ANY OTHER START DATES WILL BE REDIRECTED . COMPLIANCE CAN PEND FOR START EXCEPT INSTANT BACKGROUND, DRUG SCREEN AND LICENSE VERIFICATION.

EMR: Crown Web
Dress Code: Dialyze Direct will provide black scrub tops. Scrub bottoms: Personal scrubs of any color
No RTO allowed since this is a shorter assignment.
Non-billable orientation: 8hrs
Holiday and OT Multipliers: 1.25%
Call, Call-Back and Charge: Not applicable for this need
Requirement description :
- 1 year or more CURRENT Dialysis experience - Required
- Copy of Current, Valid Driver License - Required
- Geriatric patient experience - Required
- NXStage Dialysis Machine experience - STRONGLY PREFERRED, Please note experience on submission
- CNN or CDN Certification - PREFERRED
- Crown Web EMR experience - Preferred
- BSN - Preferred

- Work History
- Skills Checklist (pertinent to position), current to 1 year
- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications - Must be uploaded with profile
