- Skills Checklist, current to 1 year, pertinent to the specialty
- 2 Supervisory Reference Forms, taken within the last two years
- Licensure & Certifications (NBSTSA & BLS - AHA)
CERTIFICATION REQUIREMENTS :
- BLS
- NBSTSA
STATE LICENSE REQUIREMENTS :
- Oklahoma
ADDITIONAL LICENSE REQUIREMENTS :
Weekend Requirements : 1 Full Weekend/Mo
On Call Requirements : 6 Call Shifts Per Month
