Under general supervision, responsible for providing clinical/case management services to patients and their families in order to promote the optimal individual, interpersonal and social functioning of the patient and assure reintegration in to the community or the environment most suited to the patient s continued well being.
Requirement description :

• Master s degree in Clinical Social Work from an accredited graduate school of Social Work or Master s degree in counseling or other behavioral health discipline - REQUIRED
• Social Worker must be registered on the Roster of Psychotherapists who are Nonlicensed and Noncertified with the State of Vermont Office of Professional Regulation prior to employment - REQUIRED
  
• Must accept a flexible 40-hour work week scheduled to meet the needs of patients and families within the guidelines of the bargaining agreement - REQUIRED
• Experience with children and adolescents - REQUIRED
• One to two years of inpatient social work experience - preferred

• Resume with psych experience and age groups listed Ex. Psych RN - Facility - (AGE GROUP/POPULATION) - DATES
• Units must be listed on resume (Medical social worker, Behavioral health social worker, Long term care social worker, etc.)
• RTO cannot exceed 7 days
  
• Skills checklist (within the last year)
• 2 supervisory references from last 2 years of employment
• Certifications - certifications don t need to be active at time of submission but must be willing to obtain
• Driver's license or candidates address must be included in the submission packet (35 miles or less will result in rate decrease)
