The Social Worker provides services to customers and their families/significant others, to help them adjust to illness, injury, or hospitalization. Assists the health team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes. Responsible for facilitating timely, efficient, and appropriate discharge of customers and coordination with next level of care, including transmission of appropriate plan of care. Identifies needed community services and coordinates appropriate services for customers in need.

Requirements:

• 2+ Years of Hospital Social Working Experience - Required
• Acute Care (Hospital) Discharge Experience- Required
• Adult and Geriatric Experience - Required
• MSW or CSW - Would accept a BSW - $2870/Week

Certification Requirements:

• BLS from AHA
