Temp Social Worker Lmsw Hospital Days Apply
The Social Worker provides services to customers and their families/significant others, to help them adjust to illness, injury or hospitalization. Assists the health team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes. Responsible for facilitating timely, efficient, and appropriate discharge of customers and coordination with next level of care, including transmission of appropriate plan of care. Identifies needed community services and coordinates appropriate services for customers in need.
Requirement description :
- 2+ Years Hospital Social Working Experience - Required
- Acute Care (Hospital) Discharge Experience- Required
- Adult and Geriatric Experience - Required
- MSW or CSW - Would accept a BSW at the rate of BR - Required
- Epic Experience - Preferred
- Work history
- Skills Checklist, current to 1 year, pertinent to the specialty
- 2 Supervisory References with dates employed listed
- If a candidate is not eligible to work in the USA, it must be noted on their profile
- ALL RTO requests must be included at the time of submittal, NMH may not honor RTO requests after submittal
ADDITIONAL LICENSE REQUIREMENTS :
Weekend Requirements : Every 3rd Weekend
On Call Requirements : No On Call
