Join an expanding team and program to meet the needs of young adults with mental health diagnoses. Access Services is hiring a Transition Age Youth Case Manager who will join our Transition to Independence Process (TIP) program serving across Lehigh and Northampton Counties, with an office based in Bethlehem, PA.
What You'll Do:
As a Transition Age Youth Case Manager, you will join an elite team dedicated to enriching the lives of young adults aged 16-26 in your community. The TIP Model is an evidence based model that actively engages youth through individualized person centered strengths-based planning. As a Transition Age Youth Case Manager, you will work in one-on-one and in group settings, in diverse community locations. With a caseload of 15 young adults, you will collaborate with your team members to address the unique needs of each person in the program. Your guidance and support will empower these young adults as they navigate the journey to adulthood, building relationships, accessing available resources, and developing resilience.
We are seeking an individual of high character, a true team player who is passionate about a future career in mental health and social services. Your strength-focused approach, along with a non-judgmental and motivated mindset, will make you an ideal candidate for this position.
Work Schedule: Full Time, 40 hours Monday-Friday. The schedule offers a degree of flexibility, allowing for occasional adjustments based on work load demands and individual needs. Some evening and weekend hours are required for special events and occasions.
Requirements
Qualifications of a Transition Age Youth Case Manager:
- Passionate about helping their community, skilled at engaging diverse populations, comfortable navigating community settings and resources.
- Experienced working in the mental health field, and/or with transition age youth.
- Computer proficiency
- Excellent verbal and written communication skills.
- Valid license with 2+ years of driving experience with no suspensions, reliable vehicle, and car insurance (required)
Education:
- Bachelor’s degree, which includes or is supplemented by at least 12 college credits in Human Services, Psychology, Social Work, Sociology, or a related field of study.
OR
- A High-School Diploma AND 12-semseter credit hours in sociology, social welfare, psychology, gerontology, or other social sciences partnered AND at least 2 years of work experience providing direct care to people with mental health needs.
Knowledge, Skills, and Abilities:
- Excellent communication skills.
- Aptitude for setting positive, innovative, and goal-oriented objectives.
- Capacity to independently manage and prioritize tasks
- Collaborative mindset to work effectively as part of a team.
- Familiarity with creative strategies to support young adult mental, behavioral, and emotional health.
- Ability to engage well with new individuals, and develop trusting relationships
