Touchstone Health of San Antonio, a full-service Home Health and Hospice agency, is actively seeking a qualified and compassionate Transition Nurse (LVN)(RN) to ensure that our patients receive services essential to successfully transition from a skilled environment/Hospital to their homes. The ideal candidate will coordinate transitions through a variety of settings, including hospitals, transitional care facilities, skilled nursing, assisted living, and private residences. Patient-centered transitional care includes communicating with the patients and families to achieve the appropriate care plan, based on patient preferences and clinical statuses. It includes assisting with logistical arrangements, informing and educating patients and loved ones, and coordinating the services of the health professionals involved in the transition.

Requirements

Qualifications & Requirements:

  • At least two years of combined work experience as a professional nurse, preferring at least one year of which is in the home healthcare field and/or skilled nursing facility.
  • Skilled in nursing practice and interpersonal relationships.
  • Able to make meaningful observations and write comprehensive, accurate reports in a timely matter.
  • Training and/or experience in researching medical records, using internet search engines, and have a working knowledge of spreadsheets and word documents.
  • Demonstrated ability to assess and respond to the needs of patients/families in varied settings.
  • Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
  • State driver’s license and availability of car.
  • U.S. citizenship or permission from Department of Labor to work in this state.
  • Maintain a current active license to practice as a Licensed Vocational Nurse (LVN) in the State of Service. Attend all mandatory meetings and participate in continuing education programs.
  • Excellent communication skills. Multi-lingual skills are always a plus. Must be able to relate information concerning a patient’s condition.
  • Provide person-centered compassionate care appropriate to the needs of patients.
  • Be a supportive team member, contribute to and be an example of team work and team concept. Seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must have excellent interpersonal relationship skills, able to relate to and work with the ill, injured, disabled, elderly, emotionally upset, and, occasionally hostile people within the workplace/community. Deal tactfully and respectfully with others, including patients, coworkers, patients, family members, visitors, government agencies/personnel, and the general public. Have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult situations. Must be able to cope with the mental and emotional stress of the position.
  • Able to perform required tasks under standard conditions and circumstances, subject to frequent interruptions and schedule changes. Make independent decisions when circumstances warrant such action. Must function independently and have flexibility, personal integrity, and the ability to work effectively with patients, staff personnel, business and community partners and support agencies.
