Location & Work Arrangement

- Work Arrangement: 100% Onsite.
- Interview Mode: Initial Phone followed by an In-person interview.
- Shift: Monday Friday, EST.
Key Responsibilities
Youth Mentorship & Curriculum Facilitation
- Facilitate the implementation of the "Next Steps" life skills curriculum to prepare youth for independence.
- Evaluate the five mandatory "Transition Consultations" for every student to ensure individualized reentry readiness.
- Participate in weekly Administrative Release Committees (ARC) to confirm and reinforce student reentry plans.
- Conduct longitudinal follow-up calls to released youth at 1, 3, 6, 9, and 12-month intervals to monitor success and provide support.
Administrative & Documentation Support
- Assist students in navigating bureaucratic requirements to obtain essential documentation, including Social Security cards, State IDs, Driver's Licenses, and Birth Certificates.
- Collaborate on the development of an Individualized Education Learning Plan for each youth upon entry.
- Assist students with transitions back to public schools, or into employment, apprenticeship programs, and military service.
Partnership & Community Engagement
- Act as a representative for the Division of Youth Services at professional conferences and meetings.
- Assist in developing strategic partnerships with organizations such as the Department of Workforce Development (DWD), HIRE, and HOPE Mentoring.
- Coordinate with state, county, and private agencies to secure comprehensive reentry services for transitioning youth.
