Planning: Creating a transition Plan, detailing time , effort and resources need to complete the transition.

Assessing readiness: Determining the readiness of all stakeholders , tools availability , all other dependencies to create a effective plan and address any gaps

Managing the transition: Overseeing the end-to-end transition of services from Third party vendors

Tracking the transition : Monitoring and reporting on progress toward defined targets, Documenting the KT sessions, Minuting the details , highlighting any scheduling gaps

Mitigating risk: Developing risk management plans and establishing risk controls

Communicating: Communicating with stakeholders and coordinating activities between projects, suppliers, and service teams

Defining support requirements: Defining support requirements and collaborating with impacted teams to document arrangements for managing the To be system.
