🕒 Schedule: Day Shift | 5x8s (8:00 AM – 4:30 PM)
We are seeking an experienced RN Case Manager to join a leading acute care facility. This role focuses on care coordination, utilization review, and discharge planning, ensuring patients receive efficient, high-quality care across the continuum.
Key Responsibilities- Coordinate patient care plans and ensure appropriate utilization of services
- Perform admission reviews and determine medical necessity using evidence-based guidelines (InterQual preferred)
- Lead discharge planning and coordinate post-acute care services (SNF, DME, home health, transport, etc.)
- Collaborate with interdisciplinary teams including physicians, nurses, and social services
- Conduct patient assessments and participate in treatment planning
- Facilitate family meetings and provide patient/family education
- Manage referrals, authorizations, and insurance-related processes (Medicare, Medi-Cal, HMOs, IPAs)
- Maintain accurate documentation in Epic EMR
- Ensure compliance with CMS, HIPAA, and regulatory standards
- Active California RN License (or pending at time of submission, required by start date)
- Minimum 2 years of acute care case management / discharge planning experience
- Strong experience in care coordination and utilization review
- Experience with Epic EMR
- Knowledge of Medicare guidelines and healthcare regulations
- Ability to manage a high patient caseload (up to 25 patients)
- Strong communication, organization, and problem-solving skills
- Acute Care Hospital Setting
- Patient Population: All age groups (Neonates to Geriatrics)
- Fast-paced, collaborative interdisciplinary environment
- Case Load: Up to 25 patients
- Ratio: 1:20
- Every other weekend required
- Must be available to work holidays
- May cover multiple units on weekends (focus on discharge planning)
- No floating required
