Case Manager (CM) - Menino 7 West (MS/T, Stroke & Med/Surg Patients)\n\nSchedule: 4x10-hour shifts, no set day off, every 6th weekend, no major holidays.\n\nRequired Experience: 3+ years in Med/Surg with strong knowledge of MS care patterns, acute inpatient care, physical/mental impairment assessment, and healthcare plans/contracts.\n\nPreferred Experience: 5+ years in Utilization Management (UM) & Case Management (CM), experience with EPIC.\n\nRequired Certification: BSN.\n\nPreferred Certification: CCM or related.\n\nRole Overview:\nThe Case Manager (CM) collaborates to assess, plan, implement, monitor, and evaluate patient care for 18-22 patients. Responsibilities include managing length of stay (LOS), ensuring efficient resource utilization, and determining medical necessity using InterQual and Medicare inpatient lists. Effective communication and resource management are key to delivering quality, cost-effective care.
