Care Manager, Manager-Case Management , Clinic Care Coordinator-RN\n\nShift Information: M-F, 8-5\n7.30-4.30, no holidays no weekends. Maximum of 2 Days Call off (Unapproved RTO) during contract.\nMinimum Years of Experience: 3\nPrimary Care Clinic at Tsehootsooi Medical Center.\n\nTitle: Care Coordinator (case manager) nurse positions within our Primary Care clinic:\n\nRequirement: Three (3) year Case Management Experience.\n\nShift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.\n\nHousing: Information can be found under the 'Facility Group Document’ section.\nOrientation: on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation.\n
