Market Director of Case Management\nABOUT THE JOB
\nWe are seeking a motivated Director of Case Management of for a 13-week assignment at an Acute Care Hospital in Johnstown, PA.

\nQUALIFICATIONS\n\n
- \n\t
- MUST have director experience at hospital >150 bed, managing large teams (approx 20) \n\t
- Acute, LTAC, SNFs, and TCU Experience (CM Director will need to keep Transitional Care Unit full)\n\t
- MUST have EPIC experience, InterQual, discharge planning, Conditions of Participation, CMS guidelines, required. \n\t
- Will need to provide education and guidance with providers- this is a younger group who needs guidance to grow their confidence w/ their discharges, getting discharge done in the morning, etc. \n
