🏥 TCU Rehab Unit Overview
Unit Name: Transitional Care Unit (TCU)
Facility Type: Post-Acute Care (Licensed under LTC regulations)
Total Beds: 32
EMR System: Meditech
✅ Written Confirmation Required from clinician within 24 hours of offer
✅ All Titer, BKG, Drug Screening, and Fingerprinting must be completed within 3 business days of acceptance
- Charge Nurse: 1 per shift (floating RN assists with admissions, discharges, IVs, etc.)
- Nurse Aides:
- 1st & 2nd shift: 2–3 aides (based on census)
- 3rd shift: 1–2 aides
- Nurse-to-Patient Ratios:
- LPNs:
- When census is ~18: each LPN manages 9 patients
- Higher census (20–32): typically 3 nurses on day/evening, 2 on nights
- RNs:
- Charge RN oversees all patients per shift
- LPNs:
- Weekend Requirement: Yes
- Holidays: Required
- Shift Hours: 22:30 – 07:00

Patients are typically non-acute, non-critical, often in recovery from surgeries or acute illness.
- Post-Ortho Surgeries: 🦵 (Hip/Knee replacements)
- Step-down patients: From acute care, not yet ready for home or LTC facility
- Length of Stay: Avg. ~14 days
- All patients must meet skilled care criteria
