Role Overview

We are seeking an experienced Registered Nurse (RN) Case Manager to support care coordination across:

Acute Care
Swing Bed
Outpatient Services

Key Responsibilities:

Discharge planning
Utilization review
Care coordination
Ensuring CMS compliance
Collaborating with interdisciplinary teams to optimize patient outcomes

Charting System: Cerner

Requirements

Active RN License (WY) or willingness to obtain
BLS (Basic Life Support) – Required
CCM or ACM Certification – Preferred
Minimum 2+ years of clinical or case management experience
