  • Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug related problems and improving therapeutic outcomes.
  • \n
  • Provides patient care according to laws and regulations governing pharmacists and the practice of pharmacy by state and federal agencies.
  • \n
  • May prescribe medications under specific collaborative drug therapy management agreements (CDTM).
  • \n
  • Does not require direct patient care.
  • \n
  • Directs and coordinates pharmacy services activities and delivery.
  • \n
  • Supervises and verifies accuracy of work completed by support personnel during assigned shifts.

  • Doctor of Pharmacy degree required.
  • \n
  • Pharmacist State License required.
  • \n
  • Massachusetts Controlled Substances Registration (MCSR) required.
  • \n
  • Preferred 2-3 years experience working as a pharmacist.
  • \n
  • Extensive knowledge in IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution.
  • \n
  • Attention to detail.
  • \n
  • Knowledge of principles of growth and development over the lifespan (neonate to geriatric).
  • \n
  • Ability to assess patient data relative to age-specific needs.
  • \n
  • Works relatively independently.
  • \n
  • Participates in training of medical, nursing, and allied health professionals.
  • \n
  • Must complete 20 continuing education credits per year to maintain license.
  • \n
  • Familiarity with pharmacy practice settings, legal requirements, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling, and proper drug handling and storage.

  • Documentation of two measles, two mumps, and one rubella vaccines or two MMR vaccines required, or proof of immunity via blood test.

  • Documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or negative QuantiFERON TB Test or T-spot within 3 months and updated if >6 month break.
  • \n
  • Physician health statement within 365 days of start date and updated if >6 month break.
  • \n
  • Criminal background check within 45 days prior to start date and updated if >6 month break.
  • \n
  • Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break.
  • \n
  • Resume including past 7 years of work experience required for submission.
  • \n
  • Two references within 4 years of submission required for submission.
  • \n
  • Skills checklist must be completed within 1 year of submission.
  • \n
  • Confirmation of assignment/POI form required for all initial assignments and extensions.

  • Education verification through background check company.
