Minimum Requirements:

• Must have State License to Apply
\n
• EPIC experience
\n
• Experience with medication reconciliation for discharge medication list
\n
• Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
\n
• Communication skills for discharge counseling with patients/families
\n
• Clinical knowledge of antimicrobial stewardship, including interpretation of culture results for recommendations and knowledge of UTI, pneumonia, and SSTI for recommendations
\n
• Experience with pharmacokinetic dosing management/monitoring
\n
• Experience with anticoagulation dosing management/monitoring
\n
