Travel Physical Therapist Apply
Evolve Healthcare is seeking an CA Licensed Physical Therapist for a 12 week travel contract in Thousand Oaks CA!
Schedule is Day shift, 5 x 8hr shifts per week ; 40 hours per week. Contract duration will be 12 weeks. Projected start date is ASAP pending credentialing.
Week 1: M-F 8:00 to 4:30 for an 8 hour shift (30 min lunch)
Week 2: T-Sat 8:00 to 4:30 for an 8 hour shift (30 min lunch)
Compensation is $1,994 gross including a tax-free meal & housing stipend of $1,224 on 40 hours. Hourly W2 wage is $20.
Candidates within 50 miles of Facility Location, Local Rate is $1,710 at $47.50/hr

Number of Beds 296 acute beds, census for PT in acute between 100 and 120
Type of equipment ARJO, Encore lifts as needed, aide on schedule Tues-Sat
EMR - Meditech
Must have:
CA PT license
Comfortable to treat in ICU, Tele, Ortho, Med-surg
Preferred
Acute experience
Rehab (IRF) experience for ability to float
List typical procedures performed on unit(s):
Stroke, Joint replacements, Debility associated with respiratory virus, Cancer

REQUIRED CERTIFICATIONS:
