  • Active Registered Nurse (RN) license in Wisconsin or a compact state\n
  • Minimum 1 year of case management or utilization review experience\n
  • Familiarity with InterQual criteria and utilization management protocols\n
  • Experience with EPIC or similar electronic medical record systems\n
  • Strong communication and collaboration skills for multidisciplinary care coordination\n
  • Patient advocacy skills and ability to coordinate safe discharge planning (home health, SNF, community resources)\n
\n\nCase Management RN Responsibilities\n

  • Manage an assigned caseload and develop/implement comprehensive care plans in collaboration with physicians and multidisciplinary teams\n
  • Assess patient needs, evaluate treatment appropriateness, and apply InterQual tools to determine levels of care\n
  • Coordinate discharge planning and transitions of care with patients, families, and community providers (home health, skilled nursing facilities, etc.)\n
  • Monitor and review patient progress to ensure quality care and appropriate resource utilization\n
  • Serve as a patient advocate by providing education on disease management, treatment options, and post-discharge instructions\n
  • Document care coordination, utilization reviews, and discharge plans accurately in EPIC\n
