Job Summary
We are seeking a dedicated and skilled Registered Nurse for our Case Management team. The ideal candidate will provide comprehensive nursing care and support to patients, ensuring effective coordination of services across the continuum of care. This role requires strong clinical skills, the ability to work collaboratively with healthcare teams, and a focus on patient education and advocacy.

Duties

• Assess patients' needs and develop individualized care plans in collaboration with healthcare providers.
• Coordinate and facilitate communication among patients, families, and multidisciplinary teams to ensure continuity of care.
• Monitor patient progress and adjust care plans as necessary to meet changing health needs.
• Educate patients and their families about treatment options, available resources, and health management strategies.
• Maintain accurate and thorough patient records using electronic health record systems such as Epic or Meditech.
• Ensure compliance with regulatory standards and best practices in case management.
• Participate in quality improvement initiatives aimed at enhancing patient outcomes and service efficiency.

Experience

• Minimum 2 years of experience in nursing, with a focus on case management or a similar healthcare setting.
• Strong experience in patient assessment and care coordination is required.

Requirements

• Active FL State RN license.
• BLS certification required.
