Job Summary:We are seeking an experienced RN Case Manager for a contract opportunity in an acute care hospital setting in San Pedro, CA. This role focuses on care coordination, utilization review, and discharge planning, ensuring patients receive appropriate, timely, and cost-effective care throughout their hospital stay.
The ideal candidate will have strong experience working in acute care case management environments and be comfortable collaborating with interdisciplinary teams to drive patient outcomes and efficient care transitions.
Key Responsibilities:- Coordinate patient care across the continuum from admission through discharge
- Perform utilization review using InterQual or MCG guidelines
- Develop and implement effective discharge plans
- Collaborate with physicians, nurses, and interdisciplinary teams to ensure appropriate care levels
- Conduct concurrent and continued stay reviews to support medical necessity
- Facilitate communication between patients, families, and care teams
- Ensure compliance with hospital policies, regulatory requirements, and payer guidelines
- Utilize Epic EMR for documentation and care coordination
