Schedule: Monday through Friday, 8:00 AM to 4:30 PM

Requirements:
- Active California Registered Nurse license
- Current BLS certification through the American Heart Association
- Minimum 2 years of recent RN Case Management experience
- Experience in LTAC (Long-Term Acute Care) or STACH (Short-Term Acute Care Hospital) settings required
- Demonstrated competency in MCG criteria and discharge planning across acute care environments
- Strong interdisciplinary team collaboration skills
