Position: RN Case Manager
Location: Tuba City, AZ
Pay scale: $2,742 - $2,842/week
Shift: Days

Responsibilities:

• Assessing patients' healthcare needs, developing care plans, and ensuring appropriate medical care is provided.
• Coordinating patient care across different healthcare providers and settings (e.g., hospitals, outpatient clinics, home care).
• Serving as a liaison between patients, families, healthcare providers, and insurance companies.
• Monitoring patients' progress, evaluating outcomes, and adjusting care plans as needed.
• Educating patients and their families about medical conditions, treatments, and self-care management.

Requirements:

• Required: BLS, RN license
• Utilization Review experience required
• Minimum 2 years of experience
