Job Description:

• Under general supervision, responsible for providing clinical/case management services to patients and their families in order to promote the optimal individual, interpersonal and social functioning of the patient and assure reintegration in to the community or the environment most suited to the patient's continued well-being.
• Must be flexible to work across the life span with our Child/Adolescent and Adult populations.
• Provide social work, case management, therapy services to the assigned case load.
• Performs all job duties in line with Social Worker, Behavioral Health Therapist responsibilities.
• Follows all organizational policies and procedures outlined in the Policies and Procedures Manual of the facility.
