Position Name: Registered Nurse Hospice Admissions

Contract Length: 13 weeks contract -Accepting Local and Travel Candidates
Pay Rate (Days): Range $2200 to $2280 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Unit: Home Health, Hospice
Shifts available: Days
Shift timings: 10 hours; Days, 10.00 am - 8.30 pm
Schedule: 40 hours/week guaranteed, 4 X 10-hr shifts per week, Mon, Tue, Thu, Fri of each week, Wednesday and Weekends off, No call
Position Requirements:
- Registered Nurse: 2 years - Required
- Recent/Current Hospice Nursing experience in a home health setting: 3 years Required
- Must have experience with Admissions and Case Management - Required
- Home Care Home Base (HCHB) charting experience Required
- Must have a valid Driver's License and Personal Auto Insurance
- Must be Covid Vaccinated or provide a medical or religious exemption
Education, License and Certification Requirements:
- ASN or an equivalent Required
- BSN or an equivalent Highly preferred
- RN License for State of Michigan - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide and coordinate care for assigned hospice patients in comfort of their homes.
- Manage case load of up to 14-18 patients and complete admissions
- Develop and implement nursing care plans for assigned patient case load
- Support patient comfort and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians' orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients' status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.
- Provide bereavement support to patient's families as needed.
- Territories: Float to all color teams
