Position: LPN / LVN (Nursing)

Location: Utica, NY 13502

Pay Rate: $37/hr

Duration: 13 weeks contract

Shift: 8hr Eve

Guaranteed Hours: 37.5 or 40 per week

"Your pay rate depends on whether you are considered a local or travel candidate. A travel position is defined as residing more than 75 miles from the facility. Candidates who live (within 75 miles) are considered local and will receive a local pay rate."

Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse (RN) or physician.
• Administer medications and treatments as prescribed, and monitor patients for adverse reactions or side effects.
• Assist with patient assessments, including vital signs, and document patient information accurately.
• Develop and implement individualized care plans in collaboration with healthcare professionals.
• Assist patients with activities of daily living, including personal hygiene, mobility, and nutrition.

Skills Checklist:

Driver's License:

• Address on the license must match the candidate's demographics.

Certifications & Licenses:

• Current BLS Certification (must be valid).
• NURSYS Report for license verification.
• Must have an ACTIVE LPN NY License (plus any other active licenses).

Requested Time Off (RTO):

• Must be included at the time of submission (cannot be added later).

EMR Experience:

• PointClick Care experience is required.
