About the Role:
We are seeking a skilled and compassionate Occupational Therapist (OT) to join our team and provide exceptional, patient-centered care. In this role, you will evaluate, develop, and implement individualized treatment plans for patients with a variety of diagnoses, including orthopedic, neurologic, cardiovascular, pulmonary, geriatric, wound care/lymphedema, and chronic pain conditions. You will play a vital role in helping patients achieve their rehabilitation goals and improve their quality of life.
Responsibilities:
- Patient Assessment & Evaluation: Conduct comprehensive evaluations using standardized and non-standardized assessments, considering each patient's unique needs (cognitive, emotional, cultural, and chronological). Thoroughly review patient records to understand their medical history and current condition.
- Treatment Planning & Implementation: Develop and implement personalized, client-centered treatment plans focused on maximizing functionality, independence, and quality of life. Utilize appropriate therapeutic techniques and adapt treatment plans as needed based on patient progress.
- Documentation & Communication: Maintain accurate and timely documentation, including evaluations, treatment plans, progress notes, daily treatment notes, team notes, and discharge summaries. Clearly document patient education and home program instructions. Collaborate effectively with the interdisciplinary team to ensure coordinated and effective care.
- Patient Education & Care Coordination: Educate patients, families, and caregivers on managing deficits, improving daily function, and safely participating in life roles. Develop home programs to support continued progress after discharge. Participate in discharge planning to facilitate a smooth transition to home or outpatient care.
- Compliance & Professional Development: Adhere to all relevant policies, including infection control, safety regulations, and confidentiality guidelines. Maintain current CPR certification and engage in ongoing professional development to stay current with best practices in occupational therapy. Demonstrate a strong understanding of contraindications, precautions, and treatment parameters, consulting with the care team as needed.
- Caseload Management & Flexibility: Manage a productive caseload and address routine challenges effectively. Maintain flexibility with scheduling and be available for occasional unscheduled shifts with 24-hour notice.
Qualifications:
- Required: Current and valid Occupational Therapist license. Relevant experience in inpatient and outpatient care.
- Preferred: Experience working in a healthcare setting.
- Essential Skills: Strong clinical reasoning, patient assessment, and documentation skills. Ability to collaborate effectively within an interdisciplinary team. Excellent communication skills for patient education and care coordination. Solid understanding of rehabilitation protocols and best practices.
