We are seeking a skilled Pharmacist to provide high-quality pharmaceutical care at a VA healthcare facility. The role involves medication preparation, therapeutic drug monitoring, patient counseling, and compliance with VA standards. The contractor will supply all management, supervision, and labor necessary for non-personal staffing services.
Hours of Operation:-
24/7/365 pharmacy operations.

Work shifts: 8-12 hours with a minimum 12-hour rest period between shifts.

Weekend and holiday coverage required.

Lunch breaks (30 minutes) and additional breaks based on workload.

Contract: Base Year with two (1-year) option periods.

Medication Preparation & Dispensing: Ensure safe, accurate, and timely delivery of medications.

Therapeutic Drug Monitoring: Assess drug regimen appropriateness, identify interactions, and evaluate allergies/adverse reactions.

Patient Counseling: Educate patients on medication use, side effects, and compliance.

Health Status Appraisal: Conduct medication histories and patient interviews.

Medication Reconciliation:

Pharmacists under a Scope of Practice (CPP): Update medication profiles with VA and non-VA medications.

Pharmacists not under a Scope of Practice: Review discrepancies, coordinate with care teams, and educate patients.

Adverse Drug Events & Error Reporting: Document and report ADEs, near misses, and medication errors.

Medication Utilization Evaluation (MUE): Evaluate and improve inpatient medication safety and effectiveness.

Non-Formulary & Medication Approvals: Review consults, recommend alternatives, and ensure compliance with VA policies.

Therapeutic Interchange & Substitution: Implement VA policies for formulary compliance and medication substitutions.

Recall & Medication Shortage Management: Ensure alternative medication availability and compliance with treatment regimens.

Prescription Management: Manage medication orders, alerts, and prescription holds.

Preceptorship & Staff Development: Mentor students, residents, and peers; provide educational in-services.

Emergency Preparedness: Participate in emergency planning and readiness activities.

Documentation: Record all patient care activities in CPRS for accuracy and workload credit.
