Job Summary: Physical Therapist

- Evaluate and assess patients' physical conditions, symptoms, and medical history, primarily in an outpatient setting (90% outpatient, 10% inpatient).
- Develop individualized rehabilitation and treatment plans for patients recovering from accidents, surgery, or managing degenerative/terminal conditions.
- Implement therapeutic exercises, stretches, hands-on therapy (e.g., massage), and other interventions to restore or improve physical function and manage pain.
- Collaborate and communicate with other healthcare professionals (speech therapists, physicians, nurses) to ensure comprehensive patient care.
- Document patient progress, care plans, therapies, and maintain accurate clinical records.
- Educate and coach patients and families on therapeutic techniques, exercises, and care plans.
- Maintain patient confidentiality and demonstrate sensitivity to patients' developmental and sociocultural backgrounds.
- Participate in professional communication and teamwork, sharing relevant information with healthcare teams and referral agencies.
- Attend in-services, continuing education, departmental meetings, and contribute to peer performance reviews.
- Adhere to professional standards, report to work as scheduled, and perform other duties as assigned.

Qualifications:
- Master's Degree in Physical Therapy from an accredited program.
- Current state Physical Therapist license.
- Current Basic Life Support (BLS) certification or ability to obtain within 6 months of hire.
- Experience working directly with patients in a long-term care or hospital setting (less than 1 year required).
- Strong documentation, verbal communication, and teamwork skills.
- Ability to provide age-appropriate care and maintain professional relationships.
