Title: Wound Care Nurse Practitioner
Status: Part-time, transitioning to Full-time
Location: Leominster, MA
Salary: Salary to be discussed with the Recruiter

Job Summary:
Join a dedicated team delivering essential wound care services to residents of assisted living facilities, skilled nursing facilities, and home care patients across Central, Northeast, and Southeast Massachusetts. This role offers flexibility, professional growth, and the opportunity to make a significant impact on patients' lives.

Key Responsibilities:

  • Conduct thorough wound assessments and develop tailored treatment plans.
  • Provide wound care therapies, monitor patient progress, and adjust treatments as needed.
  • Collaborate with patients, families, and facility staff to ensure clear communication of care plans.
  • Accurately document care provided in electronic medical records.
  • Maintain compliance with federal and state regulations concerning wound care and patient privacy.


Qualifications:

  • Active, unrestricted Massachusetts NP (Nurse Practitioner) license.
  • Certification in wound care or significant wound care experience.
  • Minimum of 1 year of clinical experience post-graduation.
  • Possession of a National Provider Identifier (NPI) number.
  • Proficiency with electronic medical records.
  • Strong communication and team collaboration skills.
  • Ability to travel to various locations within the specified region.


Relocation Requirement:

  • Candidates must reliably commute or plan to relocate before starting the role
