This is a full‑time position that will service patients across Scottsdale and Fountain Hills. Annual salary range is $85,000 – $95,000 per year (depending on experience), plus travel expenses. Responsibilities Build meaningful relationships with patients and families. Create personalized care plans that truly make an impact. Coordinate care as a trusted case manager. Educate and empower patients and caregivers with confidence. Collaborate with an interdisciplinary team that shares your commitment to excellence. Qualifications Current RN license in the State of Arizona. Valid driver’s license and reliable, insured vehicle. Strong clinical skills and a passion for patient‑centered care.
