Job Description :
The Home Health Registered Nurse (RN) provides skilled nursing care, case management, and community health education to patients in their homes. This role involves comprehensive patient assessments, development and revision of care plans, medication administration, and coordination of services to meet individual patient and family needs. The RN works independently in the field while collaborating with physicians and interdisciplinary teams to ensure quality, continuity, and patient-centered care.
Responsibilities:- Complete initial and ongoing patient and family assessments to determine home care needs.
- Perform physical assessments and document history of current and previous illnesses.
- Develop, initiate, and revise individualized care plans based on nursing diagnoses.
- Administer medications and treatments as prescribed by physicians.
- Provide direct patient care and nursing interventions within the scope of practice.
- Counsel patients and families to address nursing and related needs.
- Provide patient education on health care instructions and preventive measures.
- Identify and implement discharge planning needs prior to patient discharge.
- Serve as Case Manager when assigned, coordinating care for the patient caseload.
- Prepare clinical notes and communicate patient status and changes to primary physicians regularly.
- Coordinate care with community health resources and other healthcare providers.
- Ensure availability of necessary equipment and services for patient care.
- Current Registered Nurse license in the state of practice.
- CPR certification.
- Graduate of an accredited school of nursing.
- Knowledge of Oasis documentation system.
- One to two years of acute care experience in an institutional setting preferred.
- Excellent observation, verbal and written communication, and problem-solving skills.
- Ability to perform nursing skills per competency checklist.
- Valid driver's license, insured automobile in good working order for field visits.
- Experience in home health or hospice nursing.
- Case management experience.
- Additional certifications related to community health or specialty nursing.
- Work involves prolonged walking, standing, and physical activities including lifting or transferring patients and equipment.
- Exposure to communicable diseases and bodily fluids is possible.
- Must be able to work under stress during emergency situations and irregular hours as needed.
- Independent work environment requiring strong time management and decision-making skills.
- Work schedule includes flexible hours based on patient needs and visit scheduling.
- Field-based role requiring travel to patient homes.
- May require occasional evening or weekend hours depending on patient care demands.
