Monitor services for your assigned client caseload. Ensure compliance with program and licensure standards. Support and collaborate with office staff. Report concerns to the Lead Program Director. Travel for client visits and community activities. What You Bring College degree in Social Work or a related field (required). 2+ years of experience providing services to individuals with disabilities (required). Case management experience (preferred). Home health experience (preferred). Familiarity with CLASS, CBA, DBMD, PHC, or DADS programs (a plus).
