  • OASIS knowledge required.

  • One (1) to two (2) years of acute care experience in an institutional setting preferred.

Position Summary

The Home Health Registered Nurse provides skilled nursing care to patients in their homes by assessing patient needs, developing and implementing individualized care plans, administering treatments, providing education, and coordinating services with physicians and the interdisciplinary care team. The nurse promotes continuity of care while supporting patients and families throughout the home health episode.

Key Responsibilities

  • Perform initial and ongoing patient and family assessments.
  • Develop, implement, and revise individualized plans of care.
  • Administer medications and treatments as prescribed.
  • Provide direct skilled nursing care in accordance with the Nurse Practice Act.
  • Educate patients and families on disease management and home care.
  • Coordinate care with physicians and community resources.
  • Prepare and maintain clinical documentation and patient records.
  • Communicate changes in patient condition and obtain physician orders as needed.
  • Identify and implement discharge planning needs.
  • Serve as Case Manager for assigned patients when designated.
  • Coordinate necessary equipment and support services.

Required Qualifications

Education

  • Graduate of an accredited school of nursing.

Experience

  • OASIS knowledge required.

Licensure

  • Current Registered Nurse (RN) license in the practicing state.
  • Valid driver's license with a reliable, insured automobile.

Required Certifications

  • CPR certification.

Preferred Qualifications

  • One (1) to two (2) years of acute care experience in an institutional setting.

Schedule

  • Monday through Friday.
